First off do NOT make fun of them, that will only make the person feel worse. I would change their whole life style, food, exercise, sleep, water intake, and daily activities.

Exercise is a good way to start, like riding a bike, walking the dog, rollerblading. The child should not think of it as exercise thought, it should be something fun to them so they will continue to do it.

The food in the house should be changed to more healthy choices, as well as smaller sized portions. The child also should be involved in which foods they like as well as cooking the meals (this will help later in life) .

The child should make sure and drink plenty of water, not soda or juice. There can be some Gatoraide to keep the body balance, but they have things like Crystal Light on the Go to help flavor the water so they will be more apt to drink it.

Above all the child would need emotional support. They need to know that someone is supporting them no matter what.

Exercise and eat right. A nutritionist can help create a diet plan that is healthy. Join a sport or being taking fitness classes (martial arts, kick boxing, cardio, yoga).

Walk to and from school when it's nice out.

Eat smaller meals and have healthy snacks.

Not eating, skipping meals, waiting long times between meals will cause the body to believe it's being starved and begin to store all food as fat to save for later.

I know someone who is 16 and weighs about 350, it's hard to make them, but have them eat smaller meals 5-6 times a day (to help metabolism) have them excercise, start off with a little, and gradually build, and eat a healthy low calorie diet

first of all its not childs fault if she/he is overweight - its parents fault. for a child to lose weight all family has to change their lifestyle - diet and fitness - replace junk food with healthy food
find daily calorie intake calculator on the net, enter all your stats and it will tell you how many calories you need a day to maintain, take off 300-400cal off that number and thats how much you need to lose.
eat 5-6 small meals a day (eat every 2-3 hours - it optimizes your metabolism)
5-8 servings of fruit and veggies a day
8 glasses of water
have complex carbs for breakfast - they give you energy
have protein (lean meat, legumes etc) for dinner - repairs muscle
cardio exercise 4-6 times a week for 30-50min (ideally 45min because the first 20-30min body burns carbs and only then starts burning fat), light weight training (more muscle=faster metabolism)
dont consume foods that are made of white flour (white bread, cakes, pasta etc.), sugar loaded foods (cookies, icecream, candy etc) and nothing fried, oily.
ofcourse you can spoil yourself once in a while with a little treat:)
