Do you mean a kidney instead of a liver? Because you have two kidneys and can donate one and still live; but you have only ONE liver, and liver donations are taken from the deceased. Kidney failure is common in diabetics, so I assume you mean KIDNEY, and not liver.
The son needs to talk to his father's doctor first to see if he is even a candidate for donation, and THEN talk about his fears.
Why DOESN'T the son want to donate a kidney? To collect the inheritance faster?

Well, that's a delicate situation... Was the son already found to be a match? If not the son can have the doctor tell the dad that his kidney will not work for the transplant... Make up some medical reason...Or the son can ease his own fears by talking to the doctor and getting good information... That might change his mind about the donation...

Only if they agree to give their organs away. Haha. I get booed for having common sense. Doctors don't rip opened dead patients for their organs unless the patient consented to have their organs donated. Murderers, pedophiles, and sex offenders should have a say, too. I don't know if one can get a brain transplant, but it's proven that at least in some pedophiles, they have a chemical imbalance in their brain, so I don't think that would be a good idea, but a murderer's kidney is not why he killed someone.

Please have your friend talk to the doctor about his fears.
They will only be taking a part of his liver.
There are some risk with any surgery but with your friend being young and healthy these should be minimal.
Many people will die each day without organ transplants just because people do not know what is involved or are squeamish or other reasons for not giving.
Every person can help.
Even you, be a donor, be tested.

Is it possible to donate a liver? Anyway, Is he seriously don't want to help his dad? Why? Wouldn't it be nice to know you helped your dad in someway? Okay, fine he's scared. I'm sure somehow the dad will understand. But if he wants to less hurt his feelings, I suggest find a better and healthier organ. So instead of saying,"I don't want to give you my liver because I'm scared" you'd say, "I don't want to give you my liver because I found a better organ donor". :) Still It's sad to know you wouldn't do that.

There is no good way to tell someone, "I don't love you enough to take this risk for you," and that's what it boils down to, whatever his reasons. The son would rather have his father die than give up part of his own liver.
I guess that's his choice to make, but maybe he should see a counselor and try to find out why he's making that choice.
Why should he want not to feel sorry? It would be appropriate to feel terrible!
