It varies depending on location & what's going on. Outlining over the spine or near the sides is rough. Shading on the arm - no big deal. My back isn't fully inked (yet), but I can speak for a number of areas on the torso & upper arms.
One of the regulars has a tree down down her spine, neck to waist. All I can think of is "ow...".
Tattooing is built up as this really painful process, and until the endorphins kick in it is a little rough. However, I like the way my artist phrases it:
"Either you've already had something this painful, or it's about time".

All depends on your own sensitivity and placement. Also, touch ups generally hurts worse. Think of getting a shot over and over again, at a rapid pace, sometimes a cluster of needles, because that's basically what a tattoo is.
The tattoo on my forearms weren't bad. They were both only one needle, about 20 minutes a piece. Not a bit deal. I got one redone when the second one was done, and the touch up hurt worse, because of the scar tissue. NOW. The tattoo on the top of my feet, both feet, just over 2 hours of work in one sitting, and boy was I glad when that was over. And the touch up was worse. I only had a few lines to redo, but 5 minutes of that and I was ready to jump off the chair.
As for the hot feeling. Everyone reacts differently to pain. The area getting tattooed of course is going to feel hot most likely because a tattoo is a wound, and the body's reaction to that is for the blood to rush to that area to seal it up, keep it clean and heal. One concern with that is getting a headrush, which it's good to take a break for a bit, or for your blood sugar to drop, which you'd need to take a break to eat something if you feel weak or dizzy and such.

The one on my shoulder blade felt a bit like something hot being drug over my skin, but I wouldn't say it hurt.
The one on my upper chest hurt in parts; close to my armpit & towards the lowest part of the tat.
The one on my lower left stomach didn't hurt, but my body didn't care for that one; I seen stars a few times.
The one on my deltoid was a cake walk.
The piece on my right thigh actually made me giggle a few times. That one was done with an air-powered tatttoo system.
The one to the right of my upper spine smarted a bit here and there.
The one on the front of my hip, little as it was, sucked at times. The outline hurt quite a bit.
Ultimately, there are a lot of factors that determine if a tattoo hurts or not....

It all depends on the area of the body that is being done if it is a meaty part of the body, a fatty part, or just a bony part. My most painful tat was on my chest and it was only over by the armpit area that hurt the fatty tissue of the chest I didn't think hurt.

it all depends on where the tattoo is located. Some of mine hurt really badly, some didn't. More bones equal more pain for me. I know that hot feeling you're talking about, but there's also a ripping feeling.

The butterfly on my pancreas hurted like hell because of the details and many colors, I found myself screaming for this guy I had a crush on, and Kind of imagine that he was putting me through that pain, but in a good way.

Of my four tattoos the one on my stomach hurt so much more it was my third one and i really wasn't expecting so much pain as the others weren't too bad.

I have seven and the one on my chest was uncomfortable but the one on my inner upper arm was 4-1/2 hours of nasty unpleasantness.

my foot tattoo didn't feel too nice. but you have to take into consideration all of the bones in your foot. my others weren't that bad. as a matter of fact, i was laughing and carrying on conversations during my leg piece and it covers almost 2/3 of my calf.

people kept telling me, oh its going to hurt so badly. and now that i think of it, none of those people have tats.
mine did not hurt AT ALL. the man was really good. it itched after a while like crazy but that was it
