My TV is connected to my cable box via HDMI. RCA jacks going from cable box to receiver...Should I bypass cable box and go from TV to receiver? Also - I do not have a surround sound set-up, just two bookshelf speakers. Guess I could always turn TV volume all the way down but it would be nice if the sounds were synced up.

TV Guy is right about the TV and receiver processing the sound differently. As a result, the only way to get rid of the echo effect you speak of is to go to the receiver from the TV rather than the cable box. This will allow you to use both the TV and the stereo system in sync. However, the TV speakers will degrade the overall sound quality (provided you have a decent stereo) and it will muddy up your stereo imaging. Still, some people with 2 channel systems like to use the TV at low volumes just to fill in the gap created by the lack of a center channel speaker.

Not possible, because there is different processing going on your TV and the receiver, so the delays may be different.

Either mute the TV or better turn the speakers off - All HDTVs allow you to turn the speakers off if you use a receiver.
