Is your Australian account still operating?
If it is, you are with the Westpac Bank formerly known as the Bank of New South Wales.
On your statement you will have a BSB number. That is the number that identifies which branch of the bank your account is lodged.
I'm sure if you contact your UK bank and provide them with your BSB and account number for the Westpac Bank they will do the transfer for you. It will probably incur a fee, from either or both ends. (That's banks for you. lol)

How long have you been over here for mate??? The bank of N.S.W. no longer exists. The name was changed years ago to Westpac.
I would get in touch with your bank in Freo and see if the account still exists as a while ago the good old Aussie government declared that if a bank account is inactive for some time that they will close that account and seize the assest. (good old government, hey?)
If you still have an active account then all you need to do is go into your Halifax Bank branch and tell them you would like to transfer money into your Aussie account and they will give you all the paper work required. You will need to know your account number and your BSB number (your bank in OZ will tell you this if you don't know it, when you call them). This will cost you around £12-£20.

Your question appears a little confused.
If you are tranferring funds from your UK Halifax account, then go into a branch and fill out a foreign currency transfer form giving details of your account in Fremantle. Dead simple.
Fremantle is in WA and by sheer obviousness, there are no Banks of NSW here.
Why should you in any event want to contact a Bank of NSW if you are transferring funds from the Halifax?

Edit Note:
From the other subsequent answers, I will point out that "many" years would seem to be something in excess of 19, as I have been here that long and never seen a "Bank of NSW". Yes indeed, I do wonder if your account still exists.

There is no Bank of NSW, it changed its name many years ago to Westpac. The easiest way to transfer funds from one bank to another is to go and see someone in either of the banks. tell them what you want to do and they will arrange it for you. That is what you pay all those bank charges for. Your UK bank can do it for you without you needing to contact the Australian bank. Your bank will also ensure that you get a key card to use in Australia.
