They moved back to the U.S. and, a year later, separated. No kids. He hasn't heard from her in four years. Was he actually married? Does he have to do something so that he can marry his current girlfriend?

If they went through a form of marriage, they are legally married. Now he has to track her down and get legally divorced.

PS: It doesn't matter where the minister was from - if he was a minister in a recognized church, then the marriage was legal. But there may be a loophole. In some South American countries, marriages may be done by a minister in a religious ceremony, but they must also be registered in a civil ceremony and yes, many people have two weddings -- the civil service the day before and then the big church service, to which they invite all their friends. '
Check with the Peruvian consulate with all the details, including the city where the marriage took place, and the name of the church and the minister, and they should be able to advise you.

A marriage anywhere in the world is legal and is acceptable to INS, the IRS, etc. BUT, usually any marriage that takes place in another country should also take place before a civil judge, or equivalent (as in my case when I flew to the Philippines to get married: first we were married in the church, then before a civil judge).
What does this person need to do to marry his current girlfriend? Get a divorce. (There's always Reno or Las Vegas, or Mexico!)

Since when is Peru in south "Africa"??!?
And yes they are legally married!!!!
