Like sometimes I'm at a grocery store and I don't want to look anyone in the eye because I'm afraid that i would have to smile back at them, because if i don't people would think I'm rude. I'm afraid to talk in front of a crowd, or any adult. At school, i can't focus on my work because I have a feeling someone is watching me, I know they aren't but just the feeling of someone looking at me scares me. I get nervous and I don't know how to react, and I start messing with my nails and fingers in a conversation. No one really knows about it, including my close friends because I act normal in front of them.
I'm afraid of even telling someone or asking for help.

You're having anxiety attacks, my dear. As a fellow sufferer, I can assure you that's there is help. I use to have an attack right before the doctor walked in the room! So don't worry about that happening, because it probably won't. And if it does, he will really see what's going on. Anyway, you must go to your doctor. There are many excellent medications out there now that really work immediately. And all doctors will believe you, because it's very common, especially with young women. (Don't we suffer enough with cramps??!!) Anyway, I believe "klonopin" is a wonder drug, but there are others that will fix you right up. I was to the point where I couldn't even have a normal conversation with my own mother, whom I adore! Girlfriend, do whatever you have to do to get a doctor ASAP. :)

I understand exactly how you feel. It's not something that you can just become "good" at - it is learned over time. Try not to be afraid. Even if you do say something stupid, it's not the worst thing. It happens even to those who have good social IQs. Sometimes socializing online is easier, as well.

I am not a psychologist, but knowing many people who have it, sometimes social awkwardness can be a sign of Asberger's Syndrome

It'll get better with time as you grow older. The more you talk to people, the easier it gets. And making eye contact is the first step. Start trying now or it'll take over your life.

Sounds like social anxiety to me. See a therapist. They have meds for it too but I would stay away from them if possible. This is something that you can usually treat with therapy. Meditation and hypnosis can often help.
