For the first couple of centimeters not all women have spotting or bleeding. It can happen so gradually that the blood vessels don't rupture. But once you really start dilating, I would say, 4-5 then yes, the vast majority of women will bleed a bit.

Well, think of it this way - when you dilate your cervix is expanding, and in the process breaking lots and lots of little blood vessels. So, yes, you probably will always spot, at least. My mucus plug had no color - or blood - when I lost it, but I did spot quite a bit while in labor.

With my first, my mucous plug was pink (I lost it all at once), with this one, my mucous plug has been yellow, pink and streaked with blood (I have been losing it in pieces). So I don't think it necessarily has to have blood in it. I have never had any spotting myself at this point.

I have two kids and didn't spot or anything when i was dilating with either one. I was dilated to a 5 when I went in to my appt, with no warning, spotting or anything.
