As a Cub fan, I believe I could give you at least 99 heartbreaking moments but since you asked I;ll give you my 3. I'm only 18, and the earliest baseball memories I have are of the 1998 home run chase, so that's all the farther I can go back.
1. Hmmmmm.....ya I guess I gotta go with the biggest choke ever.....5 outs away....our ace Mark Prior on the mound.....2003 NLCS...just 5 more outs and we're goin to the World Series. A guy named Derrek Lee steps to the plate for the Florida Marlins steps to the plate and hits a high pop up down the left field line. Moises Alou might have room......nope a fan reaches out and interferes with the play. ( I don't blame the fan for any of this.....the Cubs failed to execute plays). All hell breaks loose, our sharp handed shortstop boots a routine double play ball to end the inning and i can't go any farther....needless to say, we let the Marlins scoot past into the big show.
2. Last years World Series, Cardinals win, having to go to school the next 3 months to follow and see how happy they were. I wasn't bitter over the Cardinals winning the World Series, I was just jealous that they were so happy. I'm the only Cub fan at my entire school.....and I'm a big time baseball fan. Most of the fans are just bandwagon Cardinal fans, but they are arogant.
3. 1998, got up hopes by winning the Wild card in an do or die exciting win over the Giants but failed to show up against the Braves in the first round.
I'm sure my Grandpa could give some much bigger heartbreakers for 2 & 3. But over the last 10 years, there really wasn't much to get my heart broken over. They've only had four winning seasons since 1997 and have nearly lost 100 games 3 times.

1) 2003 ALCS Game 7...it never even should have gone to extras and Grady Little leaving Pedro in was one of the stupidest managerial decisions I have ever seen...then Boone hit that floating knuckler about a mile.
2) The great injury plagued collapse of 2006 which included a 5-game home sweep by a certain team.
3) The three and out in the 2005 ALDS at the hands of the White Sox...it didn't even look like they put up a fight.
Honestly it was tough to come up with heartbreaking moments besides 2003 even though I have been watching the Sox my whole life. It's mostly because when the Red Sox have a bad year I accept the disappointment and then move on to hope for the next season.

I am a Mets fan and I would go like this.
1. Game 6- 1999 NLCS vs. Atlanta. The tremendous comeback, down 3-0 to get to 3-2, and then down 5-0 in the game to force extras and to lose on a bases loaded walk by Kenny Rogers. I did not move out of my seat two hours after the game.
2. Game 7- 2006 NLCS vs. St. Louis. Forget the fact the Mets were 13 games better than the Cardinals. That game was very intense, lost on a one in a million Yadier Molina homerun after the Mets blew several good chances to take the lead earlier in the game.
3. Game 1- 2000 World Series vs. the Yankees. Had Timo Perez run out the ball off the wall by Zelie and Benitez comes into the ninth with a two run lead instead of one. And if the Mets win that game and at least get a split at Yankee Stadium instead of being down 2-0...........

I also think it's funny that a Yankees fan started this topic. His three biggest heartbreaks are the Kevin Brown, Carl Pavano, and Randy Johnson contracts. But the Bankees always have enough money to sign over-the-hill has-been douchebags for 25 million dollars to play in 30 games max.
Anyway, as a Cubs fan, yeah do I need to say more? We're always on the receiving end of crap like Jeter's "home run" that the little bastard kid reached out onto the field and pulled into the stands against Baltimore in 1996.
1) Bartman: just burn in hell. What's worse is they lost to the Marlins, who have no fans unless they're in the World Series.
2) 1984 NLCS vs. the Padres
3) 2004 season. Kerry Wood and Mark Prior begin their injury plagues, Sosa's fall from grace, and the Cubs blow the Wild Card lead in the final week by losing 7 of 9 against shitty teams.

I could probably name about a dozen for the Dodgers, but for me, the WORST moments were:
1. Blowing the 1962 NL pennant by losing a 4.5 game lead to the Giants with only 7 games left, including blowing a 4-2 ninth inning lead of the game 3 of the playoffs, 11 years to the day of Bobby Thomson's HR in the '51 playoffs.
2. Starting Dave Goltz instead of Fernando Valenzuela in the 1-game NL West playoff game with the Astros in 1980 - the Astros beat up Goltz, naturally, to win the division and go on to play the Phillies in the NLCS.
3. Jack Clark, 1985 NLCS, game 6. Need I say more?

#1) Willie Wilson striking out against Tug McGraw to end game 6 of the 1980 WS and giving the Phillies the title of World Champions.
#2) 1976-1978, pick a year, the Royals won 3 straight division titles and were knocked out of the ALCS 3 straight times by the Yankees.
#3) The illness and eventual death of Ewing Kaufmann. Some bad baseball people took over the team after he became ill and nearly 20 years later the Royals still haven't recovered.

Mine Are:
1. Knowing that Bernie William's career was over and that he wasn't going to play in the 2007 season was the #1 hardest thing for me as a fan.
2. Watching the Yankees game as the Red Sox swept the Yankees, 3 games to none.
3. Watching the Yankees get eliminated by the Tigers in the 2006 playoff race.

#1 Mike Soscia's HR agains the Mets in 1988. I was 7 years old and I think it made me cry.
#2 Darryl Strawberry leaving the Mets for LA.
#3 Yadier Molina's HR in Game 7 of the 2006 NLCS.
Runner up would be the Mets losing to the Yankees in the 2000 series, but lets face it, the Yanks were far superior and much more experienced then.

Being a Dodger fan all my life I respectfully submit this list to a Yankee Fan:
1- When Sandy Koufax had to retire at age 30 due to arthritis in his pitching arm.
2- When Roy Campanella was paralyzed in an automobile accident the year before the Dodgers came west and fans in LA never had a chance to see him play.
3- Last but certainly not least, the seven (7) times the Yankees beet the Dodgers in the world series. My heart actually recovered in 1955 but was broken again in 1977 and 1978.
Thanks for bringing back bad memories, but I will give you a star anyway for getting me to think.

1) The 1994 season getting cut in half. The White Sox were going back to the playoffs.
2) The loss in 1993 to Toronto in the playoffs. The White Sox should have won, but alas, did not.
3) Losing game 1 of the 2005 playoffs against LA. Sure, they won the next four, but the first one stung since LA was supposed to be tired -and it should have been an easy win.
