You're having a needle jammed through your genitals. Do you think it hurts?? And yes they freeze it - but ice on your clit is NOT A NICE FEELING.
As for horizontal or vertical - it's entirely dependent on how you feel. If your clitoris is particularly sensitive - ie direct contact can be uncomfortable - then vertical may not be for you as the bar is in constant contact with your clit. Personally I think that horizontal (a ring) looks nicer anyway.
