The first thing to remember is not to rush it. Just because she's turned 18, doesn't mean you should go down on that day and get a tattoo just because you can. I feel that anyone deciding to get a tattoo should put a lot of thought into it, because it will be on your body forever, so make sure its' something you and your sister can live with until the day you die.
As for suggestions, think of things that you two have in common, or a special occasion or thing that you two have, or a similar interest. Anything that symbolizes your "bond" to the two of you, even if you guys are the only ones that understand what the symbol means, would be great. Then it'll prompt people to ask what it means, and you can tell them about the great partnership you and your sister have together.

The ideas are endless.............you both know what is special to you............you have to make the decision.
You know how your lives have been, the fun, the cries, the hates, the loves, what you share with each other and no one else.
You have time to think about it til you sister turns 18. So, you
both have to sit down and talk about it together.

Try something that's meaningful to the two of you. For example, when my grandmother passed away, my sister got a tattoo of an orchid in her honor, since that was her favorite flower. I might get it too, not only in memory of my grandma, but to share it with my sister.

Really, no one else can tell you what to get. In my opinion, there's no point in hurrying to get a tattoo as soon as you turn 18. Instead, it should mean something to you. It should be something you'll never regret and that has some sort of significance in your relationship.

That's so sweet, but there are other ways to show your commitment to each other. Do not do something you will regret in the long run! If you still choose to get these, maybe a yingyang, or two halves of a heart.
