Affiliations are things you belong to. For example, your sports team, your church, your club....

An affiliation would be a group to which you belong, such as a trade union, that shows you have a skill or ability that would be useful to your employer. Service groups such as Boy Scouts, Special Olympics, etc. can also be good to list as they show your moral character. It is usually best to avoid church or religious affiliations on an application.
