If you go to a solicitor, you can sign a declaration stating that you no longer use the name McCarthy and you renounce all use of it, this is more commonly known as changing your name by deed poll. You may need to have your parent(s) present to do this if you are under 18. In law, you can call yourself by whatever you choose, as long as it is not to defraud somebody, therefore, you can continue using your (new) surname all the time, even without legally changing it by deed poll. The only time your birth name will become relevant is if you marry, or apply for a passport etc, you then just state your name as " Bob Jones, formally known as McCarthy" Dont worry about it, just use the name you are known by and concentrate on your exams. Once you have changed it by deed poll (it costs about £70) you need not mention the name on your birth certificate, unless you want to.
