You would either say, "yes please...I do need assistance" or you would say "no thank you". Good manners are never out of style!

It depends upon the situation. If you mean someone like a retail clerk, you might respond with, "thank you, I'd like to look around first." Are you wondering why they call you "sir?" It's only a more formal way of being polite.

Well depends on the situation .. if youre rude I guess you would say as one of the pple who answered you question suggested say I did not ask for your help, but in a shop remember its that person's duty to ask you if you need help don't think they really wanne ask you that

Say, "No thank you, I'm only browsing" (if youre in a store...I can't think of anywhere else someone would approach you in this manner). Its always proper to respond politely.

The same answer Mr Mann gave Roy in Little Britain: "If you wouldn't mind."

If you want help, as in a store, say, "yes, please, I would like (to)...." Or you could say, "yes, thank you. I am looking for..." If you don't want help, say, "no thank you, I am just browsing (looking around)."
