It still does, though various forms of heparin are more often used as injectable anticoagulants now.

Yes it still does. It is only given in the iv form to the elderly as a maintenance dosage. However in everyone else, it's given orally. Heparin is usually preferred to be given as an iv as it directly inhibits clotting as compared to warfarin that acts on the liver to reduce production of clotting factors(protein).

Yep. Still does. It's given intravenously to folks who need it, but can't swallow the tablets.

if the route of administration is IV- then you'd use heparin
if you want to give an IM medication, then you'd use lovenox
