We can't help over the net - but you can, take her to the vet!
It's common sense to take a cat to the vet for a check if she comes back after being missing. Anything could've happened to her, and it's pretty clear she has an injury of some sort and is in pain.
It couldn't be more obvious what you have to do now, so do it.

Those sore spots could be abscesses and bruises from fights she might have gotten into or try to get away from.
Take her to your Vet and get her checked out. Have her tested for feline aids and leukemia (FELV/FIV combo test). Those are two wide spread diseases that are common in outdoor cats and easily transmittable. It only takes a few drops of blood and takes about 10 minutes, so you'll have the result right then and there.
Be prepared to leave her overnight, since it sounds like she has been through quite a bit.
I also know this can get expensive. So, ask for a payment plan or go to www.carecredit.com, a low or no interest credit card for Vet bills.
Until you get her to a Vet, ASAP, keep her hydrated by giving her a little bit of water in an eyedropper every couple of hours.
Try and offer her her favorite treat (wet food, tuna...) to make her a little bit.
I hope kitty feels better soon!!!

We had a cat that had sore spots, we took it to the vet and found they were cat bites that got infected by the other cat's dirty teeth.
The vet cleaned out all the pus and had a drain in it for a few days, it was gross.
He was a stray we decided to keep until he disappeared one day :(
GO TO THE VET

I'm not a vet, but I have been owned by over a dozen cats in my life. Anytime they are in pain it's not good. Get her to a vet quickly. She may have a broken bone or an infection from a bite. Good Luck!
