If it's been going on for a month, you should see your doctor. You shouldn't be taking laxatives for that long without the advice of a doc.

The other answer is probably right, first of all, have you actually seen a doctor whilst taking all these laxatives? Secondly, the only way your rib could puncture your intestine is through trauma, i.e. don't get hit. You have to weigh up many different things ?where did you go in thailand? What did you come into contact with? If you are not feeling unwell then you are probably alright, increase the fibre in your diet (metamucil is great) cut out fat, get rid of the laxatives, unless you've got real problem you probably don't need them. As for serious issues, if you had a bowel obstruction you'd know it by now, but if you are worried, go see a doctor and get examined.

Go to a doctor for an evaluation. You may have picked up something in Thailand and might need treatment. Go to your local Emergency Room and they will be able to check you out.

You need to eat plenty of fresh fruit, fibre and drink plenty of water. Try eating fibre based cereals every morning for breakfast.
