Both.
Ella los habla - She speaks them, which could be something along the lines of:
"Ella sabe Italiano y Español?"
"SÃ, ella los habla." - Yes, she speaks them.
But if you're wanting to say "she speaks to them," you have to use the indirect object pronouns:
Ella les habla - She speaks to them.
However, not all verbs are like this:
Ella los deja. She leaves them.
Just think in English in that case if you're having troujble.

Without a context, there is no way to tell if it's wrong or not
It could be:
"Ella habla" (She talks, She is talking)
"Ella los habla" (She talks them)
"Ella les habla" (She talks to them)

It could be either one. "Ella los habla" means "she speaks them", which could be referring to languages, which would be C, but "Ella les habla" means "She talks/speaks to them". I guess it depends on the context.

As the others above said, it is incorrect. Here's why: when an action is happening TO someone, or ON someone, or sometimes FOR someone, you use le/les (or te, or me, as the case indicates!) - this is the indirect object.

Both. It depends on what you mean.
Eg:
1º
Q: ¿Ella habla esos idiomas? (Does she speak thoses lenguages?)
A: SÃ, ella los habla. (Yes, she speaks them.)
2º
Q: ¿Ella les habla a sus hijos? (Does she speak TO her children)
A: SÃ, ella LES habla. (Yes, she speaks TO them)
