I had surgery on my foot about 2 years ago. They had to do a skin graph .. It was so horrible the first time, they had to go and fix it a second time. Now the area of the skin graph is darker then the rest of my foot/toe. And the scaring is raised, and circles around it. I guess its keloids? I've tried mederma, but havent noticed any changes. any way to make it less raised, less noticable? I had a friend who had a scar sanded down by a doctor... does that work? has anyone heard of that? any other advice?

Vitamin E, like the poster above me said, and also neosporen helps to minimize scars. It won't the scar away completely, but it will make it look better, less noticeable.
Also, for the discoloration, you should use a concealer. Find one that matches the skin tone on your foot!
A lot of times scars grow less and less noticeable with age.
There's nothing wrong with scars though, they're a symbol of what you've been through and gotten over.

My mother always used cocoa butter as a solution for scar problems. Mederma doesn't sound like a bad route. Scarring takes a while to heal. Also, I know its summer and you want to wear the cute shoes, but too much sun on your new scar is a recipe for hyper-pigmentation. It will just look worse. If you just gotta wear the sandals don't forget your sunscreen.

use thius lotion called fair and white for the discoloration.and if it is raised i think you will have to consult with a dermatologist for that issue. but fair and white will durely help you. its really good.
