You should ask for a statement from the provider.

The statement should show how much you paid along with the address of the provider and their tax id number. You will need all of this info in order to claim the credit.

File Form 2441 with your tax return. You can get a credit ranging from 20% to 35% of your expenses (up to $6,000 for two childen) against taxes you owe.

If you owed taxes and did not file Form 2441, you can amend your return and get a refund of taxes you paid up to the amount of the credit.
