What is the absolute best Dental, Health ,Vision and Prescription Plan out there. They can be together or seperate plans and not necessarily together. It doesn't matter how much it would cost. I would like to know the best plans for all four that is the most complete and extensive for everything imaginable. Once again cost is not an issue as long as it is the best out there. (Plans the rich and wealthy may consider the best options)

If cost is of no concern I would recommend a Health Savings Account (HSA) Qualified plan.
1) $1,100 Deductible
2) $1,100 Max Out of Pocket
3) Rx Coverage is 100% after deductible
4) 100% Preventative / Wellness @ no cost to insured
5) Fund $2,850 into HSA
6) Gain additional $2,850 in tax deductions
7) Even if you exhaust your $1,100 deductible you have $1,750 left in your HSA to roll to next year
8) Can pay for dental co-pays with HSA tax free
9) Can pay for vision exams and materials tax free
I can go on and on - keep in mind its not the best "carrier" or "plan" its the provider network and plan design that make the best policy.
Use a local broker/agent to help you select the best plan for your area and make sure he/she offers you a PPO that gives you access to a nationwide network of doctors and hosptials.

I have Blue Shield and find it to be one of the best plans I have had. They also have a good copay on prescriptions. If you fill your generic presciptions at Wal-Mart, you will not pay more than $5 for your generic medication. It is Wal-Mart's policy not to charge more than $5 for generic drugs. They are also the most reasonable for name brand drugs, charging less than Blue Shield will pay. I also have Delta Dental, which pays about half of the dental cost. Though some vision is included in Blue Shield's plan, I have vision quest. You pay $35 and get an exam, and if you need glasses they pay for the glasses with limited frames to chose from.
It is better to associate with a group plan, as medical insurance is very costly. For me alone, it costs over $400 per month. If my retirement system did not pay this cost, I would not be able to afford it.

That's totally an unanswerable question. There is no such thing as a universal "BEST" plan. It's all dependent on your individual needs. What plan works best for me may not be the best choice for you.
My advice - research a few plans on your own and pick the one that suits your needs.

Humana and Blue Cross
We have had some really sucky insurance before and Humana is the best. They cover alot and co pays are reasonable.

There isn't one that does it all. There isn't one, also, that writes in all STATES.
Wait a second, in rereading the wording . . . you're not specifying that you want INSURANCE. You might be looking for a DISCOUNT plan (which I think is a big ripoff).
anyway, plan costs vary within a company, and they vary state to state, and coverages vary state to state and within a plan, so it's IMPOSSIBLE to compare accurately.

You'll have to go to a local, independent agent who can compare LOCAL plans for you, and help you make that decision.
