There are some HD channels, especially in the morning, that transmit a 4:3 image with black bars as HD. There is nothing wrong, this is how the image is transmitted.

Make sure your set-top is configured to output in 16:9
Make sure your set top outputs in 720p or 1080i

Go to PBS at night, that should transmit full HD.
Make also sure that your cable transmits HD in local channels even though you may not have purchased the HD-package.

In your main menu, on the remote, you should find some settings for adjusting the format , or shape of the picture, like ' wide-screen', or full-screen, etc. Once you find the settings, go through them until you find one you like. Personally, I like wide-zoom, which will have a narrow black bar at the top and bottom.
