The little purple nhs leaflet says stay 'well within the recommended limits' which is a bit vague, the NIH say caterigorically none whatsoever, and the American College of Rheumatologists say no more than 1-2 units per month. Is the odd glass of wine going to seriously mess up my liver? Is it a dose dependent thing? Is the no alcohol thing just to do with hepatotoxicity or does it interfere with the efficacy of mtx? Any opinions greatly appreciated.

I take a bone-marrow suppressant as well. I drink *very* lightly and haven't had a problem. But I also get a CBC and a panel of liver enzymes checked monthly to make sure everything is okay. I wouldn't drink at all if I didn't have these test results to show me that there is no damage being done.

I have been taking methotrexate for a few months and have not had any alcohol while on this medication. The primary reason is that both alcohol and methotrexate are toxic to the liver and combining the two could lead to a new problem - liver failure- I'd prefer to give my body a fighting chance to deal with the condition the methotrexate was prescribed for rather than risk causing further damage by having a drink.

I suppose you can, but my rheumy said absolutely none. It can cause cirrohsis of the liver. You should also be taking folic acid, which can help prevent pernicious anemia, with the methotrexate. Normally, you get a blood test every month to check your CBC and your liver function.

Maybe you'd be lucky like some of the other folks around here seem to be and have no problems. However, if you are taking Mtx, you are probably taking other meds as well and that's quite a load on your liver. Alcohol just compounds the effects.

I looked this up on Wikipedia - it doesn't list any adverse interaction with alcohol, but there were some details which make me think that more than (however much they recommend) would not be a good idea.
So I would say the odd glass of wine is fine (no more than two or three times a month) and you have nothing to worry about (except of course the chemotherapy - i do hope it works and that you make a full recovery).

I have taken methotrexate for YEARS, and I haven't stopped drinking. I don't think I drink a whole lot, but I do have a couple of glasses of wine a week. I even told my rheumatologist that I was going to do this, and he said that he would just pay extra close attention to my bloodwork to make sure my liver and kidneys were fine. I've never had abnormal bloodwork. I think it's okay in moderation, just as long as your bloodwork continues to be fine.

You can handle the truth...
Look, it depends on two thing¨s:
How long have you been drinking? That is a serious question, because we dont know how is your hepatic enzyme reserve.....
If you have already some elevation of your hepatic enzymes, such as AST ALT and GGT, STOP both the medication and alcohol...your liver is suffering and you are at risk of intoxication by both, methrotexate and alcohol.,

If your dose is not Higher that 15 milligrams per week, and used for rheumatoid arthritis control, and NORMAL hepatic tests, you can drink 2 ounces of alcohol every day.. (NOT HIGHER THAN THAT,,,,,,,)
However if the dose of methrotexate is higher than that, and your blood red cell count ius less that 11.grams per deciliter.......stop that alcohol.,...you can suffer accelerated hepatic cirrhosis...I mean it...
I have seen it with my own old eyes....

The only way to say "how much" would be by measuring the plasma level of MTX and the renal function by the time you are going to have a sip.
Since this is very... inconvenient (to say the least), we usually recommend to be off alcohol. Full stop.

You can do what you want as long as you are aware that the drugs are both toxic to liver. And taking two together increases your risk of liver damage several folds.
