Some pet supply stores on the net do sell the vaccine with tags. Most states have laws that state they will not honor a vaccine given by an owner. Some states have even outlawed the shipment of the vaccine to the public.
That leaves you with going to a vet or a rabies clinic, like at Petsmart.

Every time we get a rabies vaccination the animal gets a new tag. The vaccine and tag number are listed in the pet's medical file. There are one year vaccines and three year vaccines for rabies. But you're only supposed to get a new tag when the shot is given unless your city requires a yearly renewal of the number for their files. Ask your vet what's going on, which way the community you're in is doing this.

Every vet that administers the rabies vac has to give you the tag and write down the manufacterer and dose in the dog's medical records. If you are talking about giving it yourself then you won't be able to get a rabies tag-most states don't allow self administration of rabies vacs anyway.

You don't really need the tag, you need a certificate of rabies. Usually the vet. Although some cities have free rabies clinics.

any place that gives the vaccine will also give you the paper and the tag. Call your local shelter for most affordable place all vets give vaccine some clinic in large pet stores all will have tags

You may want to call in advance some vets will just give teh shots others will want to do an exam.
Apparently you haven't gotten on so let me tell you the pup/dog will need to get a one year booster which menas in a year you go back and get another on then they are good for 3 years.
