my friend has a car system that he knows can go over the legal limit for sound. he said they have a limit not just for disturbance reasons but because if the sub is loud enough it can actually make your heart skip if your riding inside. is it true.

There isn't any Federal "legal limit for sound", at least in the US. Different areas have various noise ordinances, but they're all based on how audible the sound is at a distance from the vehicle, not the actual SPL inside the vehicle. Most law enforcement officials wouldn't have any way to test that in any case.

Aside from hearing damage, I've never heard of a documented case where someone was injured by exposure to a loud car audio sound system.

I'd also point out that there are many industries where workers are subjected to noise levels near or exceeding what you'll find in a car audio system. OSHA regulations mandate hearing protection, and limit the time that workers can be exposed to loud noise, but don't address a danger of heart problems caused by high audio levels.

I think it's just a myth.

It can, but you will only find those SPL levels in competition vehicles with 20+ subs, 20+ batteries, 20+ amps, 2" thick lexan windows, and reinforced body panels. You have to be in the 170's for it to do damage. I have been in 160+ vehicles were it is hard to breathe, but you won't find that in an average joe's trunk.
