I don't know if this varies by state, but in Indiana, you have to have your insurance company sign the police report form. And then you have to mail it in to the police.

No, you are required to notify your insurance if you are making a claim or if the other driver is making a claim. You are required to show proof of insurance, if you choose to not make repairs through your insurance, that is your choice and the insurance company would not need to be notified.
