The one means "Do you hate me", addressed using the polite "usted".
The second one makes no sense to me. I'm a native speaker of Spanish from Spain, and at least in the Spanish spoken in Spain that makes no sense.
As far as I know, costumbre means "habit".

Do you hate me? By what custom do you direct yourself at me? The second one might be be better phrased ¿Por qué me cae así? or ¿Que tiene?
