It really depends on the amount of blood. Vomiting can be a normal reaction after surgery under general anesthesia. It's also possible that the blood is from being intubated. It could be as benign as the tube scraping her throat a little. It is also possible that is something very serious. If I were you I would just make sure the surgeon is aware of it and I would trust his opinion

Vomiting after thyroidectomy is a normal sign usually as a side effect of anesthesia. Vomiting blood however should be clearly clarified. If it looks a fresh blood in "worrisome" amount, then it may be an abnormal sign. If it is a blood stained secretions, then it could just be some blood scrapings mixed up with the secretions, then there nothing much to worry. Observing your sister for having frequent swallowing is a simple thing you can observe, as it could signal some bleeding inside. Anyway, the nurse assessed your sister and she is under close observation by the staff for post operation period so you need not to worry much.

Hopefully the nurse is right and the problem will subside. If your sister is conscious ask her what the surgeon told her before the operation.
