You will need a programmable/learning RF Remote. You will also need an RF to IR transmitter. With an RF remote the set-top box can be in a cabinet/closet anywhere in the house.

I assume the lead coming in from the dish is coaxial cable, so you connect that to "signal/cable-in" on the receiver, connect another piece of coax to " out-to- TV " on the receiver , and the other end to " cable- in " on the TV.
