about a 1/2 of an inch. if you have fast growing hair than maybe an inch.

there is no real answer to this question it depends on the way a person takes care of their hair if you cut the dead ends of every month it acctually grows faster and looks better i have alot of dead ends and need to get rid of them so and am going to be doing the same thing

the general rule of thumb when I've asked stylist and barbers is 0.5 inches a month... of course referring to scalp hair, not other hairs around the body.

This article mentions 6 inches per year (which works out to 0.5" per month), but it does depend upon genetics, age and hormones (For example, pregnancy).
