Pope Celestine V, over 700 years ago. He was literally dragged from a cave where he lived as a humble Benedictine monk and made Pope. I believe he reigned only 5-6 months before he "quit". He was later imprisoned by a Pope and died there.

The Code of Canon Law (Canon 332 §2) allows for a papal resignation:

If it happens that the Roman Pontiff resigns his office, it is required for validity that the resignation is made freely and properly manifested but not that it is accepted by anyone.

I think he died. Pope John Paul I, a real Christian. I think they poisoned his nightly hot chocolate drink.
