They want to make sure people only go to a specialist if they absolutely have to. They think that, for instance, someone with constipation will go to a gastric specialist, instead of going to the doctor and getting simple instructions to eat more fiber. Unfortunately, this can cause problems when your primary doctor KNOWS exactly what is wrong with you and won't allow you to get a second opinion, or see someone with more experience with your condition.

That depends on your insurance. If you have an HMO you have to go through your primary care doctor. If you have a PPO you don't need a referral and can go to any doctor in the plan without a referral.

Basically your insurance wants to make sure your primary doctor can take care of it first - Paying your Primary Dr is cheaper than paying a specialist to the insurance company....

For collection purposes. So if you don't pay your bill and they can't get a hold of you, they know someone who can. Sometimes the information is also for advertising as well. Who knows, maybe they really need to talk to someone in the event something were to go wrong. Highly doubt it but its usually for the first two points.
