Departure of last breath is death. Falling of a doll means that the fall has been caused by unstable conditions the doll is exposed to. Thus, both of these events are unrelated accidents.

It might mean death if it is a real big heavy doll and it lands on somebodys head. Otherwise nothing.

Dear friend,
"you create your own reality". Do not desect everything. see this as an event like any other event - like you had coffee, you went to school etc. Follow this rule of non-dissection till you have sufficient energy levels to know the meaning without doubt and at that stage you will be without fear. You can take things as they are.
