I know in my Church we not only pray for them at the Masses and on a prayer chain but we also visit them and bring them Holy Communion if they can receive it.
We also (through the priest) have the anointing of the sick. I think it is the letter of James that says "if there are any sick among you call for the priest and he will anoint you and you will get better" or something like that and it does work everytime. Even if the person dies they seem to regroup enough to have the strength to die peacefully. This used to be called Extreme Unction

One of the sacraments of the Catholic Church is the Anointing of the Sick in which the priest asks God to send His Holy Spirit to heal the person in mind, body and soul.
A good article on this subject can be found at http://www.americancatholic.org/Newsletters/CU/ac0196.asp

this covers the anointing of the sick as practiced in the roman catholic church. for people close to or on the verge of death. catholics can pray in their own words to regarding these issues whether it is a serious or minor sickness. i am unaware of any set prayer other than the priest performing the last rites. there is a part of the mass where our petitions are offered to god sometimes a person or group of people is mentioned in the petitons.
