No one has ever seen the start of a relationship, and obese blobs, (that are 50% of the population), have immature hysteria that causes delirium in good looking women.

Show some self confidence, and maintain eye contact. If you are shy, and you look down at the floor while you are talking to her, then she will assume that you are looking at her boobs and get offended. If you get so nervous that you must break eye contact, then look away horizontally, as if you are looking for someone, do not look down.
If you accidentally make eye contact across a crowed room, do NOT look away (as most guys do) until SHE looks away first. If you are too shy to maintain eye contact across a room, then it make you look suspicious and sleazy.

When you walk up to a person, in order to keep from feeling nervous, pretend like you already know her, and that she already likes you, and act accordingly. If you act nervous, she will be nervous; if you act as if she already likes you, then she probably will like you. Emotions can be contagious.
And if she doesn't like you, it is no big deal. It probably has more to do with her that it does with you. Do not take rejection personally, and you won't be nervous.

Well, if you are a guy asking a girl out, be very sweet about it. Bring a single long stemmed pink flower (pink is for admirer), rose or carnation is what I would suggest and just ask the girl. Invite her to dinner, somewhere nice, that is sit down so you can talk and get to know each other. It depends on the budget that you have available to you though.
