A year ago, a big earthquake shake my city, made me lost some relatives and friends. And I thought, I would die at that time.
After that earthquake, i realized, I did so many sins, and death can come suddenly... I don't wanna die before I've been forgiven. Then I tried to be a good Muslim, Remembered about death and day of judgment, Took salat in the early time, Learned more about Quran, Did some sunnaths.....
but day by day, I changed, back to what i used to be, spent much time in front of TV or shopping malls, lazy to learn Quran, Lazy to take pray in the early time...
I forget about death and day of judgment.
Then now, I heard rumors that tomorrow will be an earthquake that bigger than last year earthquake.
Then, again, I remember about my sins, about death, and day of judgment
Why am I always like this??
What should I do? So I can always remember about the aims of life, not only look for enjoyment?
Frankly, I'm afraid if disaster will come again
I haven't prepared to die

This isn't just a Muslim issue, but an issue for ALL people of ALL faiths around the world.
Everyone receives wake-up calls, like you and the earthquake. Some wake up and get going. Some stir, but hit the snooze button. And some just sleep right through it.
You can't change everything all at once. Start small. Perhaps keep to your prayer schedule--Muslims pray five times a day, right? Just do that for a few weeks. Then, maybe one time after prayer, start reading a little of the Qu'ran a bit at a time. This will cut into the laziness and apathy you feel too.
And do good things as you might as a representative of your Allah. Hold the door for people with their arms full. Help an elderly person with their groceries. If you see someone in pain, just go and sit with them, not to talk, but just to be there for comfort.
As they say, God, or Allah, or whomever you worship, is in the details.

The reason you feel this way is because you lost everything so suddenly and unexpectedly. You also take this blame upon yourself because you are very religious and religion has always been apart of your life. You take death and disaster as a means of coping with lost. To forget about the lost and mistakes in your life you forget about life for a moment then when you hear of a disaster you go right back from the beginning of your lost by blaming yourself and recalling the day when you saw death and lost. You are like this because you have been through a tramatic event in the past and it has not left you so easily. It's difficult to forget horrible experiences, but at the same time we must accept that life is full of sudden changes good or bad. The least you can do is try to get counseling or a religious mentor to help you through your emotional troubles. Talk to someone supportive who is close to you and will understand about what you are going through. It is almost as if you are living in fear and that is not the way to live your entire life. Try to think positive and be strong.

I don't know much about Islam, but it sounds like your question is more about simple self-betterment, so I'll give it a shot.
It sounds like you need motivation. Find a person who can motivate you to improvement, someone you will want to work hard to impress or satisfy. Maybe write down or make some art about what you're thinking now, all the reasons you want to be a better Muslim, and put them somewhere in your house where you'll see them often, as a reminder. And it's okay to start out small; just try to be a little bit better in some respect, every day, until it becomes habit. Set some sort of specific goal so you can know you kept it every day; the longer you do this specific thing, the easier it will be to do, and probably the easier it will be to move on to related good things.

Remember Allah, He will remember you. The remembrance of God is the best ... do it as often as you can and specially when you have bad thoughts in your mind. Ask for protection from Satan and his followers.
Read the Quran, with meaning, reading it without understanding is not helpful. Look around at the beauty God has bestowed on Mankind, and look away from the glitter of this world which Man has invented. This life is but an illusion, when we wake up on the Day of Judgment, it will not have been even equal to a day.

I know how you feel. And I do that too many times.....when I really need Allah's help I try to be a very good muslim and I keep that way for a time but then slowly I go back to the bad routine. I hate that so much. But we should remember death always.......that doesn't mean we cant enjoy life sometimes but we should keep up with prayers and study the Quran etc. We dont know when is our last breath . InshAllah nothing will happen but be strong in your decision to be a better muslim. we are humans so Allah knows our nature but if we even try to be good, Allah is merciful and forgiving

Do something like I did... put a religious picture at the background of your computer so whenever you get on ur computer you'd be reminded.
I've put a picture of Karbala... bcz that story and time reminds me of my duties.
Put the Quran in ur sight instead of it being with other books... I put it just beside my laptop where I work most of the time.
Put a reminder on ur cell phone for ur prayers... I sometimes get carried away while working and forget what time it is... but my cell rings and it reminds me that its prayer time (I didn't have this problem when I lived in Dubai and there was a mosque right behind our house... I could hear the Azan).
Anyways... putting a reminder for urself in today's busy life can be a good thing... you wouldn't drift off... and you wouldn't have to fear this much.

Islam shows us a moderate way. We should learn to balance our life. Pray five times, Recite and try to understand Holy Quran etc. Do all good deeds. In times of good and bad. Spending sometimes in permissive entiretinements is Ok but don't be addictive to any. If you fear something may take you away from Allah, remove that stuff from your home and don't give a thought. Sin happens to everyone. But taking sins lighly gets one into trouble. Supplicate a lot to Allah. He will always help His sincere servants.

Salaamu alaiykum, sister.
Insha'Allah, you practice the surahs you know and go from there. Then try to learn to learn surahs Ihklas, Falaq and Nas if you haven't all ready. Those surahs hold many benefits (Ihklas is eaqual to 1/3 of Qur'an) and these are the ones I recite almost as often as Al-Fatiha.
Also, try to remember that the Qur'an can help us as an intercessor after we die, insha'Allah
We all are forgetful from time time. Be constant in your "small" worship, such as dhikr.

I don't think its only you, I think its everyone..... I am sorry to say but most of the time I remember God more in the time of exams even though Y/A has helped me a lot thinking about God and afterlife... I don't have any Muslim friends where I live........ I used to have 2 guy Muslim friends but they turned out to be bigs cheating on there fiancee with this girl we all worked with in a store.... Anyway all I, we can hope for is for God to forgive us and our sins surely God is all merciful and salvation is possible only and only through him.

You are very normal At time at distress our life flashes in front of our eyes. We plan how to be better our self and truly mean it. But as time passes we sometimes revert to former practices. We all go through this. And any one that is a good faithful servant parable lacks somewhere else. Just keep trying to better yourself in little steps, not all at once. It's easier and shows your dedication.
