That is the simple one.

If you LIKE your job, put a reason why (surgery, moving, etc) and how much you appreciate everyone who you worked with.

If you don't like your job, just use the simple one.

This letter is to inform you that as of put in the date your leaving I will be leaving your place of employment and you can give a reason if you want ! Thank You Name here !! But give it to them 2 weeks before you leave !! EG... This letter is to inform you that as of June 10th I will be leaving your place of employment as I have found another job . Thank you Paul Bunion.

You can write something like this:

Dear ____________:

I am writing to formally inform you that _________(write the date that will be your last day), will be my last day of employment at ___________(the name of the firm/company)

It has been my pleasure to work with you and with the rest of the company, and I am grateful for the personal and professional growth I have experienced as a result of my employment here.

I look forward to continuing all of the friendships I have developed while at ____________(the name of the firm/company)

I didn't have to put it in writing to my job. I just went up to my boss & told her that the 15th of this month would be my last day. & she said okay.

but if you HAVE to write it ... be like

"Thank you for giving me the opportunity to work here. I enjoyed my time here and I learned a lot, but I have a new job, which is why the [date] will be my last day.

Sincerely,
[name]"

I don't know .that's just a guess.. it could work?
