Actually, it's much more forthright, and less abstract. He is being summoned back to reality. Those who are trying to bring him back are condescending to him that reality is the "cure" to his pain and suffering. He sees his "trip" as comfortable and peaceful. They see it as dangerous, painful and abusive.

He is telling them (in his mind) that he is not in pain. Then, he declares that their voices are fading... hence "a distant ship smoke on the horizon" He is simply comparing this experience to when he was a child and caught a fleeting glimpse of something. The "something" is not important at all. He is just using that memory of a fleeting glimpse to compare to his present interaction with his healers... whose voices and faces themselves are now becoming a fleeting glimpse out of the corner of his eye.
