In my Vogue Book of Etiquette (admittedly copyright 1948) I could only find one reference to refusing and that was Refusal of Food, as follows:

Soup, oysters, or any food that is served already "portioned" should never be refused. And one should take a little of at least the majority of the other courses - or, better, a little of each one. Wine is refused by a negative gesture toward the glass, just as the wine is about to be poured. The glass should never be turned down, because it breaks the rule that one must not refuse anything ostentatiously, lest it suggest disapproval (drink) or dislike (food).

Perhaps a more up to date copy of the book would be helpful.

I think it is the N because everyone can figure out that it is an O that follows..............
