I'd say Isaac Newton and Gottfried Leibnitz, who independently put together calculus from work done by a number of mathematicians. Calculus opened up areas of science and engineering that would otherwise have not been possible. Calculus allows people to tackle math problems that algebra could never solve. The first page listed below talks a bit about the practical impact of calculus.
An interesting alternative answer is Brahmagupta, a 7th century Indian mathematician who gets a lot of credit for his work in numerical analysis and popularizing the use of zero as a number. The second link gives a short bio.

David Hilbert, speaking at a 1900 international mathematical conference in Paris, set out 23 unsolved problems which afterwards had a great influence on the direction of mathematics for most of the 20th century.

Instead of 'is' in your question it might be more appropriate to use 'are' because mathematics as we know it is not the product of a single mathematician, it is the cumulative works of various intellectuals crossing all cultures. Some are more well-known in certain cultures and some not, but it is important to realise that mathematics unlike many other disciplines is an absolute institution. Therefore it is one of the most portable forms of knowledge.

I would say that Benoit Mandelbrot was one of those people he created fractal geometry which has given scientists, and mathematicians a way to measure irregular objects, and break chaos down into order.

L'Hopital! Yeah for L'Hopital's Rule!! Otherwise, Calculus would be pretty darn hard...Oh, and Pythagorus. Life as we know it would end without the Pythagorean Theorem. Let's not forget about Kramer's Rule, as well....

Grigori Perelman. Check out the winners of the Fields Medal at http://en.wikipedia.org/wiki/Fields_medal because ALL of them changed math as we know it today.
