The gums get loose, and the teeth slowly fall out. Kind of like when you were a kid and your first teeth came out.

If the lip is just cut and the bleeding has stopped, then just put some ice on it and he should be fine. If the tooth actually went through the lip (creating a hole from one side to the other) as you say in the question title, then you should probably take him to the ER because it may need stitches. Best option - call your doctor's after-hours number or nurses line and ask them (most doctors I've seen have someone you can call after hours and that number is usually given on their office's answering message) .
