How do I admit that I made a mistake to my supervisor at work. Unfortunately for me, this is the second serious mistake i've made in two months. Not sure how to go about it.

It's probably best to just tell your supervisor. It would be even better if you could provide a brief analysis of how the mistake happened, what steps you took to correct the mistake, and any steps that could be taken in future to make sure that this sort of mistake couldn't happen again. If you simply say that you made a mistake, you could simply be seen as a screw-up; if you can show how you tried to fix it, as well as show how to avoid similar mistakes in the future, you are showing the ability to accept responsibility and the ability to think proactively.

You might as well 'fess up. Otherwise when they find out about it, it'll be even worse if you tried to hide it.
