It depends on the kinds of painkillers you are taking either non-narcotic or narcotic painkillers.

Overdosing with narcotics painkillers such as morphine and pethidine and other opioid painkillers which have muscle relaxation effect could end up slowing down the muscles of vital organs. As the result, there will be respiratory depression, slowed heart functions,and all the systems will be extremely slow. So you could instantly at a very short time or while you are sleeping.

When the H+ levels go above a certain concentration, and the O2 levels fall below a certain partial pressure, the respiratory control center stimulates the diaphragm via the phrenic nerve, causing inspiration.

Narcotics like morphine, heroin, etc. disrupt this mechanism, and the body becomes desensitized to decreasing oxygen availability. The brain cells require a steady supply of oxygen, so when the respiratory system is depressed, brain cells die . . . which means you die.
