my middle child was premature weighing only 2lbs 11 oz. at birth so he did many things at a slower pace, but by the time he was 3 and he still only had a vocabulary of a few words i became concerned. i talked to his pediatrician and he recommended to have him evaluated by a local head start education group. my son started his little school program that same year and by the time he was 4 he was talking up a storm. he is now 9 and is never at a loss for words.

His parents can have him evaluated for free through the public school system. Even though not school-age, the schools work with kids who may need early intervention. My sister works at a branch of a hospital where they work with kids who are developmentally delayed for whatever reason. Autism, Aspergers, mental retardation, all sorts of things. Some kids just need a little extra work to put it all together.
It's free and the district pays for it!
Have the parents check it out, sooner than later. From knowing what my sister has said, I believe at 2 and only saying 5 words will qualify for assistance. Usually they go to the special program for mornings, or afternoons, or only a couple days a week, and do all sorts of activities designed to help. Also usually some one-on-one work if it's warranted.

Also keep in mind that often 2nd and 3rd+ kids speak later, since the older kids tend to speak for them, so it could be just a matter of that. It doesn't hurt to get it checked out, though, and can only help!

Good luck.

He is probably fine. A lot of lil boys don't talk much, and as others have pointed out, the older kids probably talk for him plus know what he wants by him pointing and grunting...

Does your nephew understand what you and others say? Does he have other ways of communicating? Can he hear? Does he make eye contact? Can he follow simple directions? If the answers to these things are 'yes,' then probably he just needs a little more time till he decides to say more.

Some children are slow in speaking; some are slow in walking. Just like adults, some are good painters, some are good in math.
How is your nephew developing otherwise?
Besides checking to see if he is getting good nourishment; plenty of sleep, etc., the only other suggestion is talk it over with the pediatrician (his parents would have to do that in most cases).

Problem might be he's only 2 years old. Some kids developer slower. If you think there is a problem, you should probably check with the doctor.

it is most likely to be his older brothers are talking for him, like was already said, as i have heard of this before. i did have a similar thing with my 2 year old daughter she was not talking very much so i took her to a ENT DR and its was found that she needed to get grommets, after she got them she is now talking really well. it might be worth a try to get his ears checked

Autism possibly. Normally the first sign is his words....if he isnt talking something is wrong. Also yes he is VERY small for a 2 yr old. My son is 7months and weights that much! Get him to a nutritionist

Is it that he just doesn't talk around you? Many kids get shy at that age and don't speak.
