The shell is formed from calcium that has been digested. The shell forms because the embryo in the egg is not developed and able to care for itself and is basically a way to protect the embryo if the egg is fertilized. It is a step up from eggs like those of amphibians or fish that must be kept moist to develop.

The shell is added at the last stage of developement.
As the ovum travels down the reproductive tract the last stage is the shell gland which deposits a calcium layer over it.
