When filling employment history out on an application.....?

what location should you choose if you've worked in a variety of positions at several locations for a company?

You would use the most recent location. When a prospective employer does a background check, that is the location that a previous employer would provide.

Definitely list the various positions that you held, but only if they pertain to the job you are applying for now.
