Panic attacks come on suddenly, without warning, not in particular settings. I would not be certain what you are experiencing are panic attacks. They seem more like phobias. You may even have both. This may partially explain why your beta blockers aren't really working that great for you. If you have phobias, seeing a therapist to learn skills to overcome the fears is the best treatment.

well, you might have to deal with these on your own in the middle of the night and you need to find a way you can cope with it to help you feel better. don't be hard on her. a lot of people are not very coherent in the middle of the night. and if she is a deep sleeper then she will have a hard time just waking her self up even to go move the couch. why though can't you just go in the bed room and put a dim light on if you just want her close to you. i would say compromise a bit on that part. but maybe try some yoga or some meditation classes to help you in the moment to learn how to relax yourself. if it comes up again and no body is around then you have to learn to how to deal with it on your own. right now, try some slow deep breathing, make some tea, turn on some mellow music at a low volume

I highly recommend getting prescribed to klonopin. This anti-anxiety medication will pretty much stop a panic attack right where it starts. It's not a narcotic or anything, and your doctor will tell you it's safe. You will definitely enjoy having many more days that are panic attack free.

She obviously cares for you very much to come over to your place at 3 in the morning!
Perhaps you should try these natural relaxation methods I have given in my sources before bed in future. If the attacks continue I would suggest going to a doctor and perhaps having medication prescribed. However, medication should be a last resort.

Don't make her responsible for your problems. There are plenty of medications available for panic and anxiety disorders. I know; I'm on one.
