By substitute I mean rental car my insurance gives me option of paying $15-$45. Of course I want to choose $45 but how much will it cost in long run?

Under no circumstances should you purchase any insurance products offered by the rental car company. Do the math...

$15/day = $5,475 annual premium
$45/day = $16,425 annual premium

Even if the car sits idle 50% of the time, the income is $2200 to $8200 a year. I don't know about rates in Mass, but full coverage where I live runs about $500-750/year. And you wonder why the rental car companies try and scare you into purchasing this coverage??? THe answer is simple... PROFIT The profit margin for these products is HUGE!.

Call your own carrier or your credit card company (AMEX or any premium credit cards). I'm sure they will all have much chaeper (or free!) alternatives

Read your policy. You may already be covered. Every policy I've had in the last 30 years covered a substitute vehicle as well as any non-commercial rental vehicle. If yours doesn't provide that coverage, you'll have to ask your insurance company how much it would cost.

What do you mean by substitute vehicle? Do you mean you borrowed a car to drive while yours is being fixed? Nothing, the insurance on the borrowed car also covers you. If you mean a second car, there is usually a multiple car discount, but ask your insurance agent.
