If you would like to be appointed by a carrier you will need to contact that carriers local sales office. From there they will provide you with appointment paper work.
If you are looking to do a AOR (Agent of Record letter for a client to a carrier), all you need to have is your client write a letter stating that you will be their new agent on their company letter head.
