I need to be licensed for Property & Casualty insurance in all states as part of my job. I was convicted of Petty Theft 12 years ago, and a DUI 5 years ago. Would this prevent me from getting an insurance license? What documentation do I need to provide on the application for the license.

Because of your convictions, In FL you wouldn't be able to get your P&C License (220/440). I would check with your manager though, because they will be able to assist you more with your situation.

If either is a felony, then yes, you won't be able to get that.
